Drug Testing Information and Requirements

Employer/Donor Instructions:

  1. Contact the testing site to make an appointment (required).
    • Please contact the testing location using the telephone numbers provided.
    • Arrive 10-15 minutes before scheduled appointment.
    • CANCELLATIONS: If you are unable to make the scheduled appointment, please contact the testing location to notify them of the cancellation.
  1. Donors must present the following information upon arrival at the testing site:
    • A photo ID with a unique identifier (example: Hawaii Drivers License, State of Hawaii Identification card, Military ID card, etc.)
    • If a photo ID is not available, a company supervisor must accompany the donor to the testing site.
    • A pre-printed laboratory form (requisition) indicating which tests are to be performed (i.e., urine and/or breath alcohol, etc.)
  1. Donors should be prepared to provide a urine sample upon arrival.
    • We recommend that the donor drink enough fluids (water, juice, etc.) to provide an adequate sample for testing. Failure to provide a sufficient sample may be considered a refusal.  

Breath Alcohol Testing ("BAT") is not performed at all locations.
